Great question.
I wouldn’t worry too much about the Moz spam score. I would use a tool like Ahrefs and assess the link profile yourself.
If it looks like you have a lot of really nasty links pointing to the site, then you will have an issue and need to do a disavow or work on getting the links removed.
My spam score is low 2% last time I looked. But if you have a website online long enough you will get poor quality links sticking to your content whatever you do.
